Case Management & Care Coordination Services
Navigating the mental health system can feel overwhelming, especially when you’re already struggling. At Therapy by the Bay, our case management and care coordination services are designed to reduce that burden by helping you access the right support, at the right time, without having to figure it all out on your own.
These services are provided by a supervised student intern and are intended to complement therapy by focusing on the practical, behind-the-scenes work that often makes meaningful progress possible.
Navigating the System is SO Hard!
And we understand that. Let one of our supervised student interns guide and educate you through processes such as finding an actually helpful psychiatrist, dietician, higher level of care program, understand your insurance benefits, and apply to programs such as SNAP and SSDI.
What Is Case Management?
Case management focuses on helping you navigate systems, access resources, and follow through on next steps outside of therapy. This can include support with applications and access to benefits such as SNAP (food assistance), SSDI/SSI (disability), Medicaid, and other financial or community-based programs. While therapy addresses emotional and psychological healing, case management supports the logistics—so you’re not left managing everything alone.
Our approach is trauma-informed, non-judgmental, and grounded in the belief that barriers to care are often systemic—not personal.
Services Offered
Care Coordination
We collaborate with your treatment team to ensure everyone is aligned and communication is clear. This may include coordinating with therapists, psychiatrists, primary care providers, dietitians, or other specialists to support continuity of care.
Level of Care Navigation (IOP, PHP, Residential)
If you need more support than outpatient therapy alone, we help you understand your options and identify programs that fit your clinical needs, insurance, and preferences. We can assist with researching programs, making calls, and facilitating referrals.
Referrals to Trusted Providers
We connect you with vetted providers and services, including:
Psychiatrists and medication providers
Eating disorder specialists and dietitians
Trauma-informed therapists and programs
Group therapy and support programs
Insurance & Benefits Navigation
Understanding insurance and public benefits can be confusing and frustrating. We help you:
Verify and understand your insurance benefits
Navigate coverage for higher levels of care
Explore lower-cost or sliding-scale options
Identify and apply for financial assistance programs
Support applications for benefits such as SNAP, Medicaid, SSI, and SSDI
Community Resource Connection
We assist with connecting you to local and national resources that support overall stability and well-being, including:
Housing and shelter resources
Food assistance programs (including SNAP)
Financial assistance and disability support (SSI/SSDI)
Transportation support
Domestic violence and safety resources
Follow-Through & Accountability Support
If you’ve ever felt stuck knowing what to do but not being able to take the next step, you’re not alone. We help break things down into manageable steps, assist with scheduling, support completing applications, and provide structured follow-up so things don’t fall through the cracks.

Services are provided via telehealth
Available to clients currently in therapy at our practice (and limited external referrals when appropriate)
